The fees, which are set by the Koninklijke Nederlandsche Maatschappij tot bevordering der Geneeskunst (KNMG, the Dutch Royal Medical Association), are established by dividing the expenses incurred per process over all applications and the work required for that purpose. These expenses include labour costs, accommodation costs, automation costs and costs for legal proceedings.
